Complete Your Forms Before You Arrive
The more we know before your child walks through our door, the more prepared we are to make their visit exceptional. We recommend completing your new patient forms online before your appointment. It takes just a few minutes and means less paperwork on arrival and more time for what matters.
Prefer to Fill Out Forms in the Office?
No problem at all. Plan to arrive ten to fifteen minutes early and our front desk team will get you set up. We’re happy to help with any questions along the way.
What to Bring to Your First Visit
- Dental insurance card and photo ID.
- Any previous dental records if transferring from another provider.
- A list of questions or concerns. We genuinely want to hear them.
- A favorite “lovey” or comfort item if your child would like a familiar friend by their side.
