How it works: Once your appointment is scheduled and you are entered into our system, our team will send a personalized link directly to your text or email.
Please complete these digital forms prior to your arrival. This allows our doctors to review your child’s history in advance and ensures we can get you back to the “Backyard” for your appointment right on time.
Preparing for the Big Day
What to Bring:
- Dental insurance card and photo ID.
- Any previous dental records if transferring from another provider.
- A list of questions or concerns. We genuinely want to hear them.
- A favorite “lovey” or comfort item if your child would like a familiar friend by their side.
